چکیده

Background: Patients with anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) require immunosuppressive therapy for disease control and reduction of relapse may be at risk complications during Sars-CoV-2 (COVID-19) infection. Objectives: To analyze the consequences COVID-19 in a large cohort AAV patients regarding occurrence, need hospitalization, treatment intensive care units (ICU), or death. Methods: Data were retrieved from March 2020 to mid-January 2021 medical records (n=233). diagnosed granulomatosis polyangiitis (GPA), microscopic (MPA) eosinophilic (EGPA) included. included age, gender, diagnosis, ongoing medication onset last follow-up non-COVID individuals. Renal involvement (ever) estimated glomerular filtration rate (eGFR) was confirmed either by positive PCR test upper airways serology. Severe defined as non-invasive ventilation, ICU care, and/or Results: The comprised 172 GPA, 50 MPA 11 EGPA. There 121 females (52%). During study period, 20 (8.6%) COVID-19. median age data retrieval all 68 years (21-93), group 63 (29-93) 68.5 (21-90) patients. Fourty-three (18%) hospitalized period which (4.7%) due In all, 8 deaths occurred 3 related At retrieval, 110 (47%) on prednisolone treatment, 10/20 (50%) 100 non-COVID-19 (p=0.5), significantly higher doses (p<0.001). COVID-19, 6/11 (54.5%) prednisolone, dose 5 mg/day (0-50). total 112 (48%) modifying anti-rheumatic drugs (DMARD) 64 (27.5%) rituximab maintenance therapy. Eight induction cyclophosphamide rituximab. Of cases, had severe these, 2 inactive without 4 stable (5-7.5 mg/day) combination DMARDs, active treated high (25-50 (n=1) (n=1). A proportion (p=0.03) then (10/213 patients). 1/8 (12%) patient therapy, compared 61/213 (28.6%) (p=0.5). present 144 (62%), 6 (30%) COVID- 19, (62%) Median eGFR did not differ between remaining renal independently Conclusion: We found infection our indicates serious complications, especially intense Maintenance seem increase findings stress continued shielding early vaccination Disclosure Interests: None declared

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis: where to go?

Enormous progress has been made during the last 25 years in our understanding of the aetiopathogenesis of the anti-neutrophil cytoplasmic autoantibody (ANCA)-associated vasculitides (AAV). This has led to improvements in early diagnosis, treatment and secondary prevention of these diseases. Nevertheless, there are still unmet needs in the AAV. With respect to diagnosis and follow-up, sensitive ...

Clinical outcomes of treatment of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis based on ANCA type.

OBJECTIVE To evaluate whether the classification of patients with anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) according to ANCA type (anti-proteinase 3 (PR3) or anti-myeloperoxidase (MPO) antibodies) predicts treatment response. METHODS Treatment responses were assessed among patients enrolled in the Rituximab in ANCA-associated Vasculitis trial according to both A...

Anti-neutrophil cytoplasmic antibody (ANCA)-associated microscopic polyangiitis following a suppurative wound infection.

life-supporting for as long as 83 days, does support contribution of xenothymus transplantation to graft survival and suggests a role for induction of T-cell xenotolerance. Therefore, while recognizing that crucial advances have been achieved, we feel that translation of necessarily highly intense and prolonged immunosuppressive regimens to human xenograft recipients will be subject to signific...

C5a and its receptors in human anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis

INTRODUCTION The complement system is crucial for the development of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). In particular, C5a plays a central role. In this study, plasma and urinary levels of C5a as well as renal C5a receptors (CD88 and C5L2) expression were investigated in patients with AAV. METHODS Twenty-four patients with AAV in the active phase, 19 patie...

Pathological classification of anti-neutrophil cytoplasmic antibody (ANCA)-associated glomerulonephritis.

What drives human beings to classify? It seems as if it is within our nature to do so. Clinical classification systems for the systemic vasculitides were composed a long time ago, and they are constantly being revised and altered. The histopathological features of many diseases are so diverse that classification is called for. The histopathological classification for anti-neutrophil cytoplasmic...
